They made them in several configurations, but the two arms on the left/one on the right was pretty common.
Normal operation would be that the right-hand control would operate the right side and the outer left arms together. The left-hand control would operate only the inner left arm.
The right-hand and outer-left arms are locked together with a key on the shaft between them, with the power to operate them coming from the right-hand cylinder. If the outer left is not operating with the right arm, the key and keyway on the shaft would be the first thing to look at. If the inner left is not working, check the fork wrist pin connecting its shaft to the left-hand cylinder. If that's in order, you're likely in for a teardown and cleanup to fix whatever is ailing that left-hand cylinder.
