OT / Computer SLOOOOOW!!!!

Ok I need some computer info from you smart computer guys!!!! I have NT on my computer and the thing is SLOW. I have a cable high speed hook. I have 76% free space, I defrag, disk clean up, run Adware, and run Spy ware at least once a week. It will speed up a little bit but not like it should be.
We crash it and it runs fast as fast can be for a while and then just gets slower and slower.
Is there something that I am not cleaning that I do not know about? What can I do to keep this thing running fast ( it can take as long as 5 minutes to log on at times). Thanks for any and all input on this matter BUDD
