I have built forms to pour those road barriers you see along the highway, septic tanks and storm sewer boxes.
Yours would be very simple. The main thing with the forms is to tapper all the sides slightly so the form slides free from the casting.
No bigger than yours is 3/4 plywood will work.
But steel does release easier. The big forms I made were all steel. You may have to reinforce the long sides with something on the outside around the top so it doesn't spread.
If it is only 4" deep about 1/8" taper is enough. You may also want to round the edges slightly on the block you put in the bottom ( for the groove) so that there are no sharp edges to lock it in there and the form will release easier.
