International Truck Motor Trouble

We have an @ 1978 International truck with a big block V8. When you get it started it will set and idle almost perfect then once in a while it will backfire and blow flames out the exhaust and go right in ideling almost perfect. When you try and move it it has no power like it should and backfireing continues. Can anyone direct me on what I can do to fix it? Thanks.
