Wheel fell off

I was using the loader on my Ford 7000 yesterday and the right front wheel just fell off. I had just had a leak fixed in that tire but they assured me that they didn't have the whole wheel off. The lug nuts are still tight on the wheel, it's just sittin there with the spindle hanging out. What happened?
