causes of accidents?

I have always had the understanding that the 2 main causes of accidents on or around tractors centered around 1) inexpirienced operators (both city people on their first tractor and farmers with a new machine) and 2) a seasoned operator who has the same thing over and over for years and it finally caught up to him. I know of a farmer who, after years of slipping the gear selector into neutral while getting off of the same tractor, was thrown because the gear selector went thru neutral and into a forward gear (floor mounted selector). Does anyone else have a different opinion on how an accident happens?
