It is always regrettable when someone has an accident, but by the definition of 'accident' - an unavoidable occurrence causing harm, or an unexpected event, - a large proportion are not actually accidents from our point of view. Many are due to taking short cuts, either by familiarity or by not considering the consequences, or by not checking out the correct way of doing a job (ignorance or inexperience?). How many times have we got away with something only because we knew what was going on before it got too much out of control?
In a lot of these fatal accidents you might consider what should have happened if a third party was injured or killed. If culpability were to be proved, then it was not an accident.
Of course, this doesn't help the victims after a fatal incident, but I hope it reinforces our need to be responsible for safety in whatever we are doing. RAB
