400 cam gear

I have a 400 farmall with a 264 gas engine. This tractor is used to pull a tedder and a NH 271 baler.The problem is,I am putting in the 4th cam drive gear in 6 years due to the nut retainer braking and the nut backig off the cam or the hub of the gear braking out completly.Any ideas on what is causing this? The last gear was spot welded to the cam so the nut stayed in place so the hub broke out. Thanks Jeff
