Jeff - Presuming the camshaft drive extension is not damaged (so the gear hub seats properly on the camshaft), the cam nut was properly torqued and the keeper installed correctly, I'd suspect the relief valve on your hydraulics is set too high. In other words hydraulic pump overloading. This in turn is overstressing the cam gear and causing your cam gear problems. Since the cam gear is located between the crank and hydraulic pump drive gears and because the gear train has helical teeth, loading the hydraulic pump causes a severe prying action on the cam gear. The top of the gear is forced forward toward the radiator and bottom is forced back toward the engine. This stress can loosen the cam gear retaining nut, and can break out the gear hub entirely as you (and many others….) have learned. You might want to remove the hydraulic relief valve and have it tested. I believe it should pop at 1,200-1,300 psi on a 400.
