Farmall A Pistons

I'm tearing down my Farmall A. In the I&T manual, it says that NEW piston to sleeve clearance should be 0.003-0.004 but does not give any tolerances for anything non new. I guess I have two questions:

First how do you take that measurement? Push the piston to one side and insert a feeler gauge? Or is it that space all the way around the piston?

Second, what is an acceptable measurement for a worn engine? I'm guessing since the pistons can move noticeably left and right that I'm going to have to replace them and the sleeves. And if that is the case, be prepared for a whole lot more questions!
