If you slowly turn the engine over by hand real slow and bring #1 up to TDC (top dead center) you would be bringing the cigarette butt and your attachment up to you. From there you may be able to draw both out with your vacuum since the piston will be up top. Take the other spark plugs out so the engine is easier to turn over. You should be able to get them out then without removing the cylinder head. Just a thought....
