My wife will generally TELL me what to get her, or I give her some money and suggest that she buy something that she really wants. I usually take her out to dinner on or close to her birthday. We get along great!
Of course, my wife knows that I don't have a lot of imagination when it comes to gifts, and she also knows I hate surprises.
