1 and 65 might mean it was made in Jan, 1965. F&H is a brand name. W1228 means it is rim 12 inches wide, 28 inches in diameter, for a tire 11-12-13 inches wide by 28inches, old size marking system. and the "W" means it was meant to go "wound and "wound for many years.
