If you follow Paul's good advise and still have the problem, another suspect is your catalitic convertor. They often do a meltdown as they die and can cause serious back-pressure. Try pulling it out of the system and check it for air-flow. If it's blocked, you should be able to look inside and see the meltdown result. Mine also rattled like it had a big ceramic rock inside.

I would follow Paul's line first. A compression test is in order. You will probably find one cylinder with much lower compression and if you're lucky it will only be a bad head gasket. If compression is relatively constant, look into that catalitic convertor.
