I don't know just how old you are, but I have the same problems at 56. A couple of things that have helped me are getting some good aerobic exercise almost every day and also being careful about my diet and losing some weight. After being borderline diabetic for years, the Doc told me that I now have Type II diabetes. But the oral medications seem to be working--and I feel better than before I started taking them. You might want to get checked out if you haven't had a physical for awhile. The doctors might find something and might be able to make you feel a whole lot better. I have forgotten why you are disabled. But living with a disability is very taxing and often leads to depression. Most depression is treatable and the anti-depressants can really help with the symptoms, one of which is having trouble getting anything done. Ageing is a bit**! and not easy for most people. But it does seem better than the alternative. Good luck!
