I am not an expert on diesels so take this with a grain of salt. It sounds like maybe you need to have the injectors cleaned and serviced, and there may also be some issues with the pump. Also, none of those older diesels like to be just putted around, they were designed to be worked hard. You may just need to put the 'ol gal on a heavy load and make her work until the temperature comes up and then keep her there for awhile.
