A bad spark plug wire, or spark plug.

Even if the wires have been replaced, you could have a bad one. We had had bad ones right out of the box, even.

Check each wire individually, from end to end, with an Ohmmeter. The shortest wires should read something like 5000 to 7500 Ohms, the longest ones, maybe as high as 10,000 or 12,000 Ohms.

Look at the plugs, to see if one looks different from miss-fire. Look for cracked insulators or other damage.

If the plugs and wires are OK, I would do a compression test, and then there are all the possibilites of injector problems, or manifold air leaks.
