Do I have a problem with trans. on 87 GMC 7000?

I just bought a 1987 GMC 7000 dump truck with a 5 speed trans. it makes alot of noise in all gears except 5th. also when you take it out of 5th and try to go back it grinds and will not go like it is not syncronized , it will go to 4th real easy and then back to 5th. with no problem.
