I've used a Hi-Lift jack. Set the foot of the jack against the tire sidewall and the lifting point against a lug on the inside tire. Try to get the jack angled in so the foot is getting as close as possible to the rim. It might take a couple of tries to get it to hold but once you get it to grip it'll push the tire right off. Be careful though. When it gives the jack will fall out. I've dropped it on my toe a few times.
