I'm not much of a holstein guy, but if she's 100 percent holstein, I'd say she should be bagged up way more than that if she's closing in on calving. Ussually they have way more milk than a single calf can possibly drink to start out with.
Who knows. If a bull came and went, or she left and came back, you'd of surely known that unless left un-attended for awhile. They ussually gone for atleast 2 or 3 days for that, and ussually don't return to where they came from. Maybe if being fed corn every day, they'd return at feeding time.
A big animal vet would be able to check her for pregnancy. Tell ya if she is bred or not. Not many raise a holstein heifer and not breed her. Seeing an un-bred one this size, is un-common. Most are raised for replacement milk cows.
