I put mine in as soon as I get up in the morning and wear them till bedtime. The tinnitus is real bad some days and having them in seems to reduce it even when it's quiet. I have the full ear molds so I can shut them off for noisy work and they will block most of the sound. Workman's comp covers the cost and I purchased the sound streaming system for the TV. They're good but I still have trouble understanding speech and have to rely on a good deal of lip reading.
