Smith Form-A-Truck

I know this is probably a Model A conversion,but will ask here for info as this is a some what unique story. We were replacing a leaking water service to a house today when we dug up a truck frame and chain drive rear end. The axle bearing caps were labeled SMITH FORM-A-TRUCK CHICAGO.The hubs still turned on the axle,nothing like good American bearings and lots of grease! Can anyone tell me more about what we found today?
