I had a rental cabin,guy fell asleep on couch and his head pushed the pillow over the arm and onto the electric baseboard and caught fire, instead of using the fire extinguisher 10 ft away he grabbed the foam rubber pillow and carried it out the front door dripping foam fire thru the house and badly burnt his hands--he was a past fire chief and sued me for 1 million cause i didn't have signs that the electric baseboard was hot--insurance co settled for $7k just to get rid of the suit
second was when i was director of bridges--guy ran thru the warning gates as the drawbridge was opening, he jumped the 10 ft opening and crashed landed on the other side and got killed--family sued saying the traffic signal wasn't red and warning bell not working. had to prove in court the interlock system on a drawbridge only allows the next step after previous steps are verified electrically---basically yellow light for 5 sec with warning bell, red light for 5 sec, gates down,locking drawbar pulled, then span can raise. they lost!
