The lever slides a splined collar forward to lock into the transmission shaft. the pin on the lever may be worn off, or the splines sometimes wear. Another possibility is if the seal retainer at the front of the transmission could be reversed allowing the shaft move forward and disengage. You can see some by removing the filler plug, but you cannot see the pto from the transmission cover. Part of the housing is between the gears and the pto.
