Ask Natasha Richardson's family. She was rich and at a high class ski resort and had to be hauled in an ambulance over a hundred miles for an MRI because there where none closer and there was NOT an AIR flight close either.
Also it was funny when the leader of the legislature in Canada needed emergency heart surgery they took him to Detroit as none of the Hospitals there could/would do the surgery.
