When I was in vet school we used a twitch some times. It was a wood handle about 24" long (like a broom handle) with a looped chain on the end. We would grab the upper lip and nose put the chain around it and gently twist. We would apply just enough twist to get the horse to obey.
Downsides, some horses lip and nose were hard to get a hold of to apply the twitch, some horses would start throwing their head if you applied too much pressure, some horses it just flat didn't work so we would sedate them with xylazine. Sucked when a horse would throw it's head and get the twitch out of your hands. Then the handle became a dangerous obstacle to dodge.
