I don't have a pic of mine, but its made from a tool handle with a hole drilled through it for the 3/8 smooth rope. They can be made with very small chain, too. I've never used the chain type.
I don't like them either, but like you said, sometimes they are necessary for the good of the horse. I think I've needed mine once.
You can purchase a twitch called a "humane twitch". It looks sort of like a weird pair of pliers. I've never used one so I don't know how well they work.
