The step stool with an eye bolt in the top and a few feet of clothsline attached worked well for an old fellow I knew. I watched him open the door and pick the little stool off the drivers side floor, set it on the ground and get in, then pull up the stool by the cord and place it on the passenger side floor
To get out, he lowered the stool by the cord and stepped out, then set the stool on the driver side floor. Simple and it worked.
