I'm guessing you're talking about an H/M style water pump and fan.
Without all of that stuff, you turn only the fan with the engine running. With all of it, the impeller turns, too. The springs help keep the impeller shaft in it's proper position. Brass or bronze nut holds packing to keep water from oozing out.
