I've got a friend who has one, 03 and a 6 speed, uses it as a farm truck and for hauling equipment, and other than expected maintenance items he's had no trouble. If you put a major power increase to them they have a tendancy to blow head gaskets. You can replace the powerstroke-specific parts with the corresponding Navistar parts for a little more power and fuel efficiency.
