Assuming you put it on the splines before you put it in, and assuming you assembled the disk in the Pressure Plate correctly, the only thing left is alignment of the disk within the clutch. Did you use a clutch alignment tool to center it. If done by eye, and you are practiced at it that is OK. If you just put it together, it is misaligned. That disk must be centered or the pilot bearing stub on the clutch shaft will not go in the pilot bearing. My best guess. Let us know. JimN
