You may be lucky and just have a lot of slop in the external linkage. Remove the cowl from above the dash and remove the platform. Examine each and every part from the end of the lever down to the top of the range transmission cover. Replace anything that is even questionable. Once you've got it to the point where there is absolutley zero slop in the linkage, adjust the shift linkage, clutch linkage, dump valve, transmission brake, and TA linkage exactly by the book. It should now shift like new. If it still jumps out of gear, then you'll need to work on the parts underneath the range transmission cover, which really isn't a hard job.
