I have to echo RAW's comments. The ball bearings have not been holding up well. I'd go with an original style graphite bearing. Be sure to lube it before installation. These things hold up for 50 - 60 years.

On the shifter there are a couple of possibilities. Either the shifter tip has worn or the tower guide is worn. Either can be repaired fairly easily.
The tip on the shift lever can be welded up and ground down and if you copy and paste this link it will show you a great shifter repair slideshow.
