Need info on PTO cover...H

My brother in law is about done restoring a early H for my father in law. We have been having a hard time finding a PTO cover that will work.

The link provided will show a picture of the rear end.
The measurements are as follows.
3 mounting holes.
The two lower ones are fairly level to each other spaced 5 3/4" apart CL.
If you go vertical from the lower left hole 7" and across to the right 3 1/2" you locate the only top hole. CL

We are looking for a good one to purchase if someone has one. Any information will be greatly appreciated. We have tried Rice's and a couple other vendors without luck
