Happened again. Twice. But to a different injector. After watching the horror of antifreeze filling the cylinder because the copper sleeve came out with the injector, I stopped pulling injectors. A fuel filter failed so I replaced the filters and flushed out the contaminated fuel. The injector pump couldn't purge the obstruction from the impaired injector because the back wash from piston emptied the tube.
So to make the engine run on the remaining cylinders, I had to removed the injector tube, run a hose from the pump outlet to the fuel tank. Hooked up a borrowed injector tester to the impaired injector and pumped while the engine ran. After about an ounce of pumping I reassembled the system and now it works great. Knock on wood.
