I've seen some small (under .250) reamers that have a pointed head and a notch on the shank, rather than a square head. How can they be used by hand? Most of my tap handles have a square opening, but one has a cross type (+) opening. I'm not sure what that one is for.

If the flat is cut on the shaft, (not at the very end), then it would be a power ream . These are usually used in a milling machine. If the flat is cut at the very end of the shaft then the proper handle opening will be shaped like a "tear drop" with a set screw coming through the rounded side and pressing the round side of the ream shaft into the wedge shaped part of the opening.
Reams with a single flat are usually of high quality and the handles are expensive machinist tools.

I wanted an adjustable reamer for holes under 1/4", but can't find any. I guess they're too small. So I found these: 370075438159 eBay.

Grind a square on them. Or maybe put them in a drill chuck on a handle ?
