First place I"d look is the connection between the cable and the clamp. That area takes a lot of flack and, after a while, it comes loose or many of the individual strands break. Either way, it"ll get hot. If it looks at all doubtful, chop a couple of inches off the cable and remake the connection.

It is the result of a bad connection. Either the clamp is not making a good connection, ie dirty clamp or weak spring. Or the cable termination in the clamp is dirty/loose. Try cleaning it all up, etc and if no luck get a new ground clamp.

I recently noticed the same thing on my cracker box.
However I had just finished a project and havnet done any checking into it. First things I thought of were-
1) tighten screw that holds cable on clamp.
2) clean clamp where it contacts the metal 3) yank the cable and install a larger one.
4) buy a 200amp wire feed mig welder.
