If the starter was spinning rather noiselessly when it was not engaging the issue may be one of stickieness of the lubricant on the spiral drive splines. To make it crank reliably, I would clean the drive splines with brake clean spray, then let it dry. I recommend then lubricating it with a product called Tri-Flo. Putting this spray on the spiral splines will leave behind a dry film of teflon platelets which are slippery on metal, and do not collect additional dirt/dust. If the starter drive did make a grinding tooth tearing noise when it was spinning and not engaged I believe the application of the above treatment in addition to cleaning up the teeth with a file is in order, as mentioned correctly below.
