Does anyone know what the intake and exhaust valves should be adjusted to when COLD? The manual states 0.020" hot, but nothing about cold.

I seem to remember that you can also turn it in to touch, then back off 2 flats. I don't have those notes with me so maybe you or someone else could confirm. I just did that math based on a 7/16" diameter, which I got from the parts book, and I think it is a coarse thread, which would make it a -16 pitch, so each turn of the nut would yield .0625", so two flats would set the clearance at .02083, which really helps when you have old rockers that have a depression worn that won't allow a flat surface to use your feeler gauge properly.

If those numbers are wrong, please to unregard!

Yep, tighten down while turning the push rod until you feel resistance, then back off two flats, or 1/3 turn. This is the best way to do it if the tappets are worn. This is straight from SM-2004, page 60-10-10.
