On my 2003 2500HD, I replaced all pads and rotors last year at ~46,000 miles. I bought the truck 5 years ago with 36K on it so I am not sure how it was maintained / driven before I purchased. (The truck only has 48,000 on it now as I drive my car to work daily.) The front rotors were completely rusted and I had a very soft brake petal. I have a co-worker with an 2001 GMC 3500 and he said his rotors are the same condition and he drives his to work daily. I believe he is right around 100K miles now and will be replacing the rotors the next time he works on the brakes. From what I found researching the net at the time, it appeared that this is not out of the norm. I also gathered that it didn't matter if you bought your rotors at the dealer or aftermarket, it really just depends on which castings you end up with as to how fast they will flake apart so I just went to the local Advance Auto and bought what was in stock. As I recall, it was around $300 for the parts. Good luck with your project.
