Well first off on a C221 the sleeves can stand out of the block up to .006 or be flush . Now maybe that sleeve moved up at one time and stuck there or maybe when it was overhauled they did not get the contour bore cleaned and did not get it all the way down ??? As to milling the head you can take off what ever you need to dress it to get it to clean up . Also spend the extra bucks and get the head gskt from Case I H as the ones you get from the parts store will fit BUT they will not seal as they are not quiet the same .
