As to the guy losing his finger. We had a guy who was cleaning a machine, stuck his finger part way thru a hole in a guard. Transfer bar behind it took his fingernail off. Engineer doing the safety report asked "this hole" Operator said yes, engineer stuck his finger in, got the end of it taken off. Then showing his boss how he got hurt, stuck his other index finger in, removed the nail on that one. They took him off the floor after that. He quit soon after. We missed him. Not too many guys make everybody feel smart.
