The measurement is taken for both width and diameter at the point where the tire bead seats against the wheel. One of the best ways to understand it is to look at a mounted tire with the size imprinted on the tire. Measure the diameter of the wheel to find points on the wheel that have the corresponding dimension. Tire width is typically about an inch more than the wheel width.
