My wifes dad died 3 years ago, Bladder cancer due to smoking. (he died a horrible death) My MIL smokes too, she quit (sort of) a year ago after being in the hospital for 6 weeks.
I told my wife when we met 20 years ago that her parents would quit smoking when they died, rude but true. You have to make basically assume that no one will live past the age of 65 if they smoke, I only know of 2 women and no men that have lived beyond 65 and smoked. The sheer cost of the smokes themselves would make me quit not just the health risks.
