The key bit of information here is the fact that things are OK as long as the lever is up on the cap. That type of cap releases the pressure when the lever is up so the system works OK with no pressure. You have a combustion leak into the cooling system. At this point the leak is so slight that it does not leak unless there is pressure in the cooling system to force the coolant into the engine.
With pressure on the system the coolant is leaking internally causing the some of the coolant to be replaced by exhaust gasses. When the coolant level drops a little the flow through the heater will be interrupted as the air passes through.
With no pressure building in the cooling system because the lever is up there is not enough pressure to force coolant into the cylinder. Most likely the cause is a failing head gasket again. Cracks tend to leak more than what you are describing.
