You don't have to fill that form out. They will just keep bugging you till you make an appointment to let someone come into our house as happened to me till they do it, Gov people will come in and ask questions and put that info in their laptop and be on their merry way.
But they did say it was a law even on the original form to have to give the information they wanted.
