I know exactly what you are talking about because I got "stuck" at that juncture also. Took me forever to figure it out.
The way to break out of that box is to get out of the octive.
Run your scales across three octives and always play your lead lines across multiple octives.
This forces you to play horizontially vs vertically and gives the voice more variation also.
Try it; you'll be surprised at how fast it comes. Just a day or two and one song played this way will lead to another before ya know it.
THEN, after you've crossed that boundry, start playing off the thirds and fifths. The listener "thinks" he's hearing the same thing in his mind even tho he's not, but it really makes it easy to just "grab" any note and run with it (in either direction) because it multiplies the scale by 3.
