Case diesel cold start

I have a '72 case 580 CK. Last winter it started acting up when I start it in temperatures less than 32 deg. As I start it, it seems to slowly build up to an RPM that will sustain itself and run on it's own. After it gets to the point where it's running, when I press the accelorator, it doesn't rev up any higher and thick black smoke comes out of the exhaust. It takes quite a while for the engine to get to a point that it is useable. It gets worse the colder it gets. It has not done this in the previous years before. I have tried using the anti-gel additives but it doesn't seem to make a difference. I do have a block heater and plug it in for about an hour before trying to start it. What can I do to get it back into its original state?
