I have had that exact same problem for several years now. Stuck in 2nd and delayed to 3rd. comes and goes can't make it do it. letting off the gas usually lets it go on into gear. My efforts at finding the problem all told me a seal in the 2nd pack leaks and won't build enough pressure to go to 3rd. Tranny needs rebuilt. I have put off the rebuild and have so far lived with it. Sounds like now yours has crapped out and needs the rebuild ?
