Ah ha! That is what I finally came up with too. Had to call three different license branches to get that answer! Printed the form off from the IRS web site so now I just need to know where to file it. Does it go to the IRS office? Can't get a real person on the phone there and if you finally do they don't know anything and send you on another trip through recorded options. Thanks for the help!
